Observation

The Individual Health Care Plan for a child was not clearly visible the document was blurry and hard to read. It was unclear if the child has an allergic reaction to the foods and dairy listed or if it is an intolerance. Another plan ready medications that are given at home but it is unclear if the medications are for the food allergy or seasonal allergies. Provider is to follow up with parent and obtain adequate and clear paperwork. The program did not have any medication forms and if the child is allergic to foods and requires medication this must be at the program with all required paperwork filled out and signed by the parent and the doctor.
